Portable Exam Kit Lending Policy:
Shipping
Shipping is possible, however due to the size and weight, pickup is preferable. If shipping is necessary, the AAO will cover the cost of shipping to the optometrist. The optometrist is responsible for the cost of return shipping, which must be done via a courier that provides a tracking number. A valid credit card number must be provided at time of booking. The Kit must be returned on the first working day after the event of its use.
The kit is checked before and after each use, if any items are missing or damage has occurred, the last borrowing optometrist will be contacted. If it it is determined that the borrowing optometrist is responsible for the missing or damaged item, the repair or replacement cost will be charged to the credit card number supplied.
Appropriate Use & Restrictions
(1) The Portable Eye Exam Kit may not be used for school screenings. The AAO does not condone screening tests in schools. Screening can give a false sense of security, causing some parents to ignore the necessity of a complete eye exam.
(2) Use of the Kit for a charity event (i.e. Hope Mission, Mustard Seed, etc.) is appropriate in most circumstances.
(3) Use of the Kit for care home visits is acceptable only in the cases of patients who are immobile and unable to access eye care elsewhere.
(4) A valid credit card number must be provided in case of damages to the kit.
(5) Any damages to the Kit while on loan with an optometrist are the responsibility of the optometrist. The AAO will charge the optometrist responsible via an invoice, which must be paid within thirty (30) days of receipt.
